A general physical examination should be performed to detect conditions that could contribute incontinence such as edema, arthritis, dementia, neurologic disorders. Special attention should be paid to the rectal examination for perineal sensation, sphincter tone, and rectal mass. Direct observation to detect stress incontinence via the cough stress test can be performed by having the patient cough vigorously while being observed for urine loss from the urethra. This test should preferably be done with the patient’s bladder full, in either the lithotomy position or, if necessary, while standing. Postvoid residual volume should be measured within 5 to 10 min after the patient voids in a comfortable and private setting.
