Established eosinophilicendomyocardial disease usually causes progressive heart failure from diastolic restriction and/or valvular dysfunction. Surgical treatment typically involves resection ofendoventricular fibrosis and atrioventricular valve replacement. However, little is known of the clinicopathological behaviour andperioperative course of this disease in the early stages. Mitral valve repair seemingly offers an attractive surgical option in this scenario as it avoids prosthetic thrombosisâ arecognizedlate complication of this disease. This was attempted in conjunction with left ventricularthrombectomy in a patient with acute hypereosinophilia and congestive heart failure associated with severe mitral regurgitation and restrictive cardiomyopathy. In the operative management of mitral regurgitation (MR) associated with aortic valve disease, a transaortic approach combining the bowtie mitral valve repair with replacement of the aortic valve appears to offer a less invasive and technically simple, expeditious alternative to conventional left atriotomy and Carpentier style repair. Methods: Between February 1997 and December 1999, six patients underwent a bowtie repair of the mitral valve via the aortic root with concomitant aortic valve replacement.

Mitral regurgitation is a common valvular abnormality that generally increases in prevalence and severity with age. Current treatment of severe mitral regurgitation requires the use of cardiopulmonary bypass and mitral valve repair or replacement. Mitral valve surgery has a significant attendant morbidity and mortality. Currently, there are two methods in development to treat mitral regurgitation by percutaneous methods. These involve a modified Alfieri technique using an edge-to- edge repair, and posterior mitral annuloplasty via the coronary sinus. Human trials will soon begin.
