FOOD FOR THOUGHT: Outcomes of Planned Homebirths...
2008-02-07 05:45:00 Results: 655 (12.1%) women who intended to deliver at home when labour began were transferred to hospital. Medical intervention rates included epidural (4.7%), episiotomy (2.1%), forceps (1.0%), vacuum extraction (0.6%), and caesarean section (3.7%); these rates were substantially lower than for low risk US women having hospital births. The intrapartum and neonatal mortality among women considered at low risk at start of labour, excluding deaths concerning life threatening congenital anomalies, was 1.7 deaths per 1000 planned home births, similar to risks in other studies of low risk home and hospital births in North America. No mothers died. No discrepancies were found for perinatal outcomes independently validated. Conclusions: Planned home birth for low risk women in North America using certified professional midwives was associated with lower rates of medical intervention but similar intrapartum and neonatal mortality to that of low risk hospital births in the United States. ACOG's Latest Anti-Homebirth Press Release
2008-02-07 04:55:00 ACOG has just recently released another statement opposing homebirth. Click here to read the full text of this press release. I have several issues with this press release and even moreso, with the medical care provided by members of ACOG. Read my objections below and feel free to post comments.I think its inappropriate and unethical to list maternal choice first as a factor in the increaseing cesarean rate. I do think that maternal choice is a factor but what about iatrogenic factors such as non-medically indicated inductions, the inability to correctly interpret data printouts from electronic fetal monitoring systems, physician impatience, and the lack of physician discretion in allowing convenience cesareans. A cesarean section is major surgery- cesearean birth, as opposed to vaginal birth, is associated with an increased rate of maternal mortality. Therefore, why isn't there more regulation surounding it? For example, patients desiring gastric bypass surgery or sex-reassignment... More About: Press Release , Press , Release , Anti

FOOD FOR THOUGHT: WHO Infant Feeding Recommendations
2008-02-05 01:58:00 FOOD FOR THOUGHT: Once a week, I will provide snippets of policy from respected organizations such as the World Health Organization (WHO), American College of Obstetricians and Gynecologists (ACOG), American Academy of Pediatricians (AAP), and many more. Some of the policies may seem counter to accepted norms, and that's why I've named the series "Food for Thought ."WHO Recommendation on Infant Feeding :"Breastfeeding is an unequalled way of providing ideal food for the healthy growth and development of infants; it is also an integral part of the reproductive process with important implications for the health of mothers. As a global public health recommendation, infants should be exclusively breastfed:(1) for the first six months of life to achieve optimal growth, development and health.(2) Thereafter, to meet their evolving nutritional requirements, infants should receive nutritionally adequate and safe complementary foods while breastfeeding continues for up to two years of age or ...
